Yes it is a real item. Look up Unreal Hood in the legendex - Rogue in your example. The eternal version comes with Mythical set, Legendary Mythology affix and 4 empty sockets. Use any hood/helmet recipe or resource system to craft an eternal Mythic. After you create the Mythic, use topaz to add double epic affix values. Most players get trolled by RNG for their Epic affix selections.
